PATH Program
The Projects for Assistance in Transition from Homelessness (PATH) Program provides adult case management services to Burlington County residents who are 18 years of age or older, have a serious and persistent mental illness, and are homeless or at risk of being homeless.
The PATH Program provides:
- Case management
- Outreach services
- Transportation
- Systems advocacy
- Linkages
Individuals may be referred by a county agency or hospital or may be self referred. Once the initial contact is made, the referral is assigned to a PATH case manager who will meet with the individual and complete the referral.
